DUTIES & RESPONSIBILITIES
1. Under the supervision of the Director of Business Development, effectively de liver the cli ni cal responsib ilit ies and asso cia ted operational responsibi lities.
2. Maintain performan ce and produ ctivity standards , operational effi cien cy , ex cel len t customer
servi ce and best pra cti ce standards.
3. Treat a cute and routine medi cal conditions via in person , telephone call or telehealth a cross all
settings (e. g SNF , ALF and house call visits )
4. Demonstrate criti cal thinking , expert care, and best pra cti ce standards in the clini cal decision-
making pro cess.
5. Provide care for pa tien ts from initial history taking , cl ini cal assessmen t, diagnosis , prescribi ng
medi ca tions and treatmen ts , providing edu ca tion, and evalua tion of care.
6. Maintain complian ce with the established standards of pra cti ce of medi cine for Nurse in the state of pra cti ce.
7. Do cumen t findings and a ctions appropria tely in the pa tien t’s medi cal record.
8. Maintain pa tien t confiden tiality a t all times.
9. Par ti cipa te in professional dev elopmen t a ctivities and maintain profession al affi lia tions and CM E’s.
10. Maintain safe work environmen t for self , pa tien t, family, and staff by using appropria te PP E
11. Work collabora tive ly with Elite team memb ers and community par tners such as ALF and SNF staff
members , pa tien t family members and home health and hospi ce team members
12. Represen t Elite in a professional manner and strive to promote a cooperative a tmosphere a t all times
by a cting with ta ct and diploma cy and maintaining a positive a ttitude.
13. Maintain professional ism a t all times by bei ng dependable, autonomous, and responsible.
14. Prioritize wor kload and utilize work time properly and produ ctively.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
Must have graduated from an approved school of nursing and be licensed in the state where practicing.
Maintains a current CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Self-directed and able to work with minimal supervision.
Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
Wage: $52 - $60 per hour
