Community Care Physicians – Community Care Family Practice Great Oaks
Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join CapitalCare Family Practice Great Oaks, a well-established primary care practice providing comprehensive care to patients of all ages and committed to coordinated, high-quality, patient-focused care.
As an Advanced Practitioner at CapitalCare Family Practice Great Oaks, you will deliver full-spectrum family medicine services in a collaborative, team-based environment. You will practice to the full extent of your license with strong physician support, integrated clinical services, and scheduling flexibility designed to promote work–life balance and long-term professional satisfaction.
Conduct comprehensive patient evaluations and manage acute and chronic conditions
Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed
Provide preventive care, health maintenance, and patient education
Collaborate closely with physicians, nurses, and support staff to ensure continuity of care
Maintain accurate, timely clinical documentation in accordance with organizational standards
Care across the lifespan – serving pediatric, adult, and geriatric populations
Flexible scheduling options, including shared extended hours (evenings/Saturdays) to support patient access and provider balance
Telemedicine and patient portal integration, enhancing continuity and convenience for patients
Established, collaborative care team with experienced physicians and Advanced Practitioners
Strong community presence with a stable, growing patient panel in the Capital Region
Compensation: starting at $125,000
CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.
