Nexus Neurorecovery Center – Conroe | Conroe, TX

Nexus Neurorecovery Center – Conroe is seeking a Nurse Practitioner to support our adult patient population with complex medical, neurological, and behavioral needs. We welcome Family Nurse Practitioners, Psychiatric-Mental Health Nurse Practitioners, and Nurse Practitioners with experience in Physical Medicine & Rehabilitation.

Candidates must be comfortable caring for adults with behavioral challenges and collaborating with an interdisciplinary team to address the connection between physical health, cognition, behavior, and recovery.

Position Summary

The Nurse Practitioner provides assessment, treatment, and ongoing clinical management for adult patients in collaboration with physicians and the interdisciplinary care team. This role supports individuals whose neurological conditions may be accompanied by cognitive impairment, behavioral dysregulation, psychiatric symptoms, and complex medical needs.

The ideal candidate brings strong clinical judgment, patience, and a practical, compassionate approach to caring for adults who may have difficulty communicating needs, regulating emotions, or participating consistently in treatment. Assigned responsibilities will align with the provider’s education, certification, clinical privileges, and scope of practice.

Key Responsibilities

  • Perform comprehensive assessments of adult patients, including medical histories, physical examinations, and evaluation of relevant cognitive, functional, and behavioral concerns.
  • Develop and adjust individualized treatment plans in collaboration with physicians and the interdisciplinary team.
  • Manage acute and chronic conditions within the provider’s specialty and scope of practice.
  • Order and interpret diagnostic testing, prescribe medications, and monitor treatment effectiveness and adverse effects as appropriate.
  • Evaluate changes in patient condition and coordinate timely treatment, specialty consultation, or transfer to a higher level of care when indicated.
  • Help identify medical concerns that may contribute to behavioral changes, including pain, illness, medication effects, and sleep disturbances.
  • Collaborate with nursing, rehabilitation, behavioral health, and case management professionals to support consistent approaches to patient care.
  • Participate in rounds and care conferences, connecting clinical treatment with rehabilitation goals and everyday functioning.
  • Educate adult patients, families, and caregivers about health conditions, medications, treatment plans, and ongoing care needs.
  • Maintain timely, accurate documentation and contribute to patient safety and quality improvement initiatives.

Qualifications

  • Master’s or doctoral degree from an accredited Nurse Practitioner program.
  • Current Texas RN and APRN licensure.
  • Current national Nurse Practitioner certification appropriate to the assigned patient population and responsibilities.
  • Family Nurse Practitioner or Psychiatric-Mental Health Nurse Practitioner background, or Nurse Practitioner experience in PM&R or rehabilitation.
  • Comfort and willingness to work directly with adult patients who exhibit behavioral challenges.
  • Strong clinical assessment, communication, and interdisciplinary collaboration skills.
  • Experience caring for adults with brain injury, neurological conditions, psychiatric disorders, or complex rehabilitation needs preferred.
  • Ability to build rapport with patients who have cognitive or communication limitations and adapt care to their individual needs.
  • Current BLS certification and applicable prescribing credentials for assigned duties.
