Job Description

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Conduct in-home or facility-based visits to hospice patients for initial assessments, face-to face visits, routine follow-up visits, and symptom management.

· Collaborate with the Medical Director or Associate Medical Director and the IDT to develop, implement, and revise individualized care plans.

· Provide expert pain and symptom management consistent with hospice philosophy and guidelines.

· Serve as a clinical resource to hospice nurses and other staff.

· Participate in IDT as needed.

· Educate patients and families on disease progression, end-of-life expectations and hospice treatment options.

· Document all patient care accurately and timely in the electronic medical record.

· Participate in on-call coverage as needed.

· Comply with company policies, procedures, and guidelines.

Location: Bryan, Texas

Shift: PRN- up 12 hours per week

Requirements

· Minimum 2 years of NP experience

· Master’s or Doctoral degree in Nursing from an accredited program

· Current and unrestricted Nurse Practitioner license in the state of Texas

· Certification as a Nurse Practitioner (AGNP, FNP, or ANP preferred)

· Current DEA Registration and prescriptive authority in good standing;

· Valid Texas Driver’s License, automobile insurance, and reliable transportation are required
