Nurse Practitioner-Palliative Medicine Expansion (Full-Time)

Seeking a diligent and compassionate Nurse Practitioner for our Palliative Medicine Outpatient team. Bilingual a plus! As a Nurse Practitioner, you will be responsible for providing consultation and follow-up visits for our community palliative medicine program. You may meet with patients in a variety of settings: Office/clinic, other physician offices, assisted living facilities, nursing homes, subacute rehabs, and patients own homes for our home bound patients and provide telehealth visits. Travel is required daily unless doing telehealth visits.

We see patients in Burlington, Mercer, Camden, Gloucester, Atlantic, and Ocean Counties. We schedule in clusters based on area where patients live. This full -time opportunity offers $108k-135k based on experience! Plus mileage remibursement.

What You’ll Do:

  • Provides a health assessment appropriate to the visit being performed, as well as diagnose and treat patients who require management of symptoms and goals of care conversations. This includes prescribing controlled drug substances as required.
  • Collaborates with patients, families, and their primary care and consulting physicians to provide excellent patient and family centered care.
  • Develop and maintain internal and external relationships, while working with our support staff to expand the outpatient reach to all communities/populations and collaboration with our Inpatient pall med team members and other programs within our organization to provide continuity of care.
  • Collaborate with all or our programs, Primary Care at Home, Palliative Medicine inpatient, Hospice, and Social connections.
  • Full time is 40/week with afterhours call rotation (telephone calls and telehealth remote-no visits)

What You’ll Bring to Our Team:

  • Prior experience preferred in palliative care, oncology, heart failure, geriatric care, and/or hospice and home care
  • RN, APN license in NJ
  • DEA and CDS license will be required
  • Must live or willing to travel to the Camden or Gloucester Atlantic County Areas OR Burlington, Mercer, Ocean Counties.
