Job Description

The Nurse Practitioner provides continuing and holistic healthcare service to individuals and families.

Essential Duties and Responsibilities:

  1. Delivers a variety of healthcare services, including preventative, chronic and acute services.
  2. Performs diagnosis and treatment of illnesses, carries out health-risk assessments, routine check-ups, screening tests and immunizations, and counseling to individuals on ways to maintain a healthy lifestyle, and to prevent diseases.
  3. Ensure participation in continuing education and training to keep abreast of current developments in the industry.
  4. Provide quality care to patients by following the most current evidence-based guidelines and procedure of practice.
  5. Takes part in quality assurance audits.

REQUIREMENTS

Nurse Practitioner License in State of Illinois

Current Illinois Controlled Substance License within 6 months of hire

Eligible for DEA registration Possession of a current DEA Registration within six months of hire

Current ACLS and BLS certification

• Licensed and certified as a Nurse Practitioner (NP) in the state of Illinois

• Board-certified in Family Medicine, Adult-Gerontology, or related field Prior experience in primary care
• Strong clinical assessment, communication, and documentation skills
• Commitment to providing high-quality, compassionate care

Bilingual IN Spanish strongly preferred
