
Nurse Practitioner – Skilled Nursing Facility (SNF)
Full-Time | W2 | Weekdays | On-Site
$150,000–$180,000 annually (DOE) + Full Benefits
We are hiring an experienced Nurse Practitioner (NP) to provide high-quality medical care in Skilled Nursing Facilities (SNFs) This is a stable, full-time W2 position offering competitive compensation, strong clinical autonomy, and a predictable weekday schedule.
This role is ideal for an NP who enjoys post-acute care, values collaboration, and is comfortable managing patients independently in an SNF setting.

Key Responsibilities
Perform comprehensive patient assessments in SNF settings
Diagnose and manage acute and chronic medical conditions
Conduct routine patient rounds
Collaborate with physicians, nursing staff, and care teams
Order and interpret labs, diagnostics, and treatments
Document all patient care accurately in the EMR
Qualifications
Active Nurse Practitioner license (state-required)
Experience in Skilled Nursing Facilities, post-acute, or long-term care required
Strong clinical judgment and decision-making skills
Ability to work independently while collaborating with a care team
Comfortable managing a patient panel in an on-site setting
