We are seeking a Cardiovascular Nurse Practitioner with at least one year of outpatient cardiology experience to support a growing cardiology service line. This role works collaboratively with cardiologists and clinical staff to provide high-quality, patient-centered cardiovascular care in an outpatient setting.
Responsibilities include:
Conduct comprehensive cardiovascular assessments, including history, physical exams, and risk stratification
Manage and follow patients with cardiac conditions such as hypertension, heart failure, coronary artery disease, arrhythmias, and hyperlipidemia
Order, interpret, and review diagnostic tests (EKGs, echocardiograms, stress tests, Holter monitors, labs)
Develop and implement treatment plans in collaboration with cardiology physicians
Prescribe medications and adjust therapies in accordance with evidence-based guidelines
Provide patient education on disease management, lifestyle modification, and medication adherence
Document clinical encounters accurately in the electronic medical record
Coordinate care with referring providers, imaging, and ancillary services
Participate in quality improvement initiatives and clinical protocols
Required
Active Nurse Practitioner license in NY
Minimum of 1 year of outpatient cardiology Nurse Practitioner experience
Board certification (ANCC or AANP)
Active DEA (or eligibility to obtain)
Strong knowledge of cardiovascular disease management
Excellent communication and clinical judgment skills
Preferred
Experience in outpatient cardiology or multispecialty practices
Familiarity with EPIC or similar EMR systems
Experience managing high-volume patient panels
