Salary Range: $125,000.00 - $160,000.00 Salary

POSITION SUMMARY

The Practitioner will creatively, consistently and compassionately use their passion for treating patients from diverse and vulnerable communities to provide high quality, evidence-based, integrated primary care, examine patients, review medical histories, interpret diagnostic tests, diagnose patient injury/illness, provide appropriate treatment and education and consult with their supervising or collaborating provider when needed. The NP/PA will document treatment appropriately in the Electronic Health Record and follow the Health Center's policies and procedures regarding quality care and compliance.

ESSENTIAL FUNCTIONS

  • Responsible for conducting individual patient health history, physical assessments and screenings to include various diagnostic testing as needed.
  • Responsible for interpreting the results and providing feedback to patient and/or designated family member.
  • Responsible for prescribing/performing various treatment modalities - including, but not limited to medications, injections, wellness counseling, suturing, and ancillary testing.
  • Responsible for development, implementation and evaluation of treatment plans.
  • Consults and collaborates with a variety of health care professionals and makes referrals as necessary.
  • Monitors and ensures patient satisfaction.
  • Facilitates appropriate utilization of the healthcare system and encourages appropriate follow-up care.
  • Provides educational opportunities for the patient and/or family related to health status, utilizing additional resources when indicated.
  • Responsible for facilitating patient's participation in medical and health care by providing information needed to make informed appropriate healthcare decisions.
  • Maintains accurate, legible, timely documentation of patient encounters.
  • Serves on and actively participates as a member on committees as assigned. Attends a variety of meetings, discussions, conferences, seminars, and workshops, both within and outside the organization as applicable to own position's responsibilities.
  • Maintains certification in compliance with current State law.
  • Able to demonstrate the knowledge and skill necessary to manage and provide the care appropriate to the ages of patients served on his/her assigned practice setting.
  • Incumbent must demonstrate knowledge of the principles of growth and development relative to the patient's specific age. Additionally, incumbent must be able to provide the care needed as described in the Health Center policies and procedures.

ESSENTIAL FUNCTIONS

Core Responsibilities (All APP Roles)

  1. Clinical Care a. Provide direct patient care including assessment, diagnosis, treatment, and management of acute and chronic conditions b. Perform comprehensive histories and physical examinations c. Order, interpret, and follow up on diagnostic tests d. Prescribe medications and treatments in accordance with state and federal regulations e. Deliver preventive care, including screenings, immunizations, and health education f. Develop and implement patient-centered care plans
  2. Productivity & Access a. Maintain expected patient volume consistent with organizational standards b. Provide same-day or urgent access as needed c. Ensure timely follow-up and continuity of care
  3. Documentation & Compliance a. Complete accurate and timely documentation in the electronic health record (EHR) b. Close encounters within organizational timeframes c. Adhere to all regulatory, compliance, and billing requirements (e.g., UDS, HEDIS)
  4. Team-Based Care New Bedford Community Health a. Collaborate with physicians, nurses, medical assistants, behavioral health clinicians, and care coordinators b. Participate in care team huddles and case conferences c. Delegate appropriately and support staff development
  5. Quality & Performance Improvement a. Participate in quality improvement initiatives b. Meet clinical quality metrics and performance benchmarks c. Support population health management efforts
  6. Patient Engagement a. Provide culturally competent, trauma-informed care b. Address social determinants of health in care planning c. Utilize interpreters and support services as needed

REQUIRED EDUCATION & OTHER CREDENTIALS

Qualifications

  1. Education & Certification a. Graduate of an accredited Nurse Practitioner, Physician Assistant, or Nurse Midwifery program b. Current Massachusetts licensure (or eligibility) c. National board certification (e.g., AANP, ANCC, NCCPA, or AMCB for CNMs)
  2. Additional Certifications (as applicable) a. DEA registration b. Buprenorphine waiver (for OBAT roles) c. HIV certification or experience (preferred for HIV track) d. Psychiatric certification (for PMHNP roles)
