Job Description

Kingsway Arms Nursing Center is seeking a compassionate, clinically skilled Nurse Practitioner to join our medical team. This role is integral to overseeing and supporting the delivery of high-quality care and services for approximately 160 residents receiving both long-term and short-term rehabilitation care.

The Nurse Practitioner will work collaboratively with physicians, nursing leadership, rehabilitation, social work, dietary services, activities, and other interdisciplinary team members to promote resident-centered outcomes, timely clinical decision-making, and regulatory-compliant care.

The Nurse Practitioner will provide advanced clinical assessment, diagnosis, treatment, care planning, and ongoing medical management for nursing home residents in accordance with New York State scope-of-practice requirements, facility policies, professional standards, and applicable federal and state regulations.

Key Responsibilities

  • Conduct comprehensive and focused resident assessments, including admission, routine, acute-change-in-condition, post-hospital, and follow-up evaluations.
  • Diagnose and manage acute and chronic medical conditions in collaboration with attending physicians, medical directors, specialists, and other members of the healthcare team.
  • Develop, implement, evaluate, and update individualized plans of care.
  • Prescribe medications, treatments, diagnostic tests, therapeutic interventions, and referrals within the Nurse Practitioner’s New York State scope of practice.
  • Respond promptly to changes in resident condition, helping to prevent avoidable hospital transfers when clinically appropriate.
  • Participate in medication review, infection prevention, wound care oversight, pain management, behavioral-health support, and other clinical quality initiatives.
  • Collaborate closely with nursing staff to provide clinical guidance, reinforce best practices, and support resident safety.
  • Participate in interdisciplinary care-plan meetings, family conferences, discharge-planning discussions, and other resident-centered care processes as needed.
  • Ensure timely, accurate, and complete clinical documentation.
  • Support compliance with Centers for Medicare & Medicaid Services requirements, New York State Department of Health regulations, facility policies, and standards of professional practice.
  • Contribute to quality assurance and performance-improvement activities, including review of clinical trends, adverse events, hospitalizations, infections, falls, wounds, and other quality indicators.
  • Foster respectful communication with residents, families, staff, physicians, consultants, and community partners.

Requirements

Qualifications

  • Current, unrestricted New York State license as a Registered Professional Nurse and Nurse Practitioner.
  • Current DEA registration and prescriptive authority, as applicable.
  • Clinical experience in long-term care, skilled nursing, post-acute care, geriatrics, internal medicine, family practice, rehabilitation, or a related setting strongly preferred.
  • Strong assessment, diagnostic reasoning, communication, documentation, and care-coordination skills.
  • Ability to work effectively as part of an interdisciplinary team and build collaborative relationships with residents, families, and staff.
  • Knowledge of geriatric syndromes, chronic-disease management, post-acute rehabilitation, medication management, infection prevention, and regulatory expectations in a nursing-home setting preferred.
