Hospice Nurse Practitioner

As a Hospice Nurse Practitioner, you’ll contribute to our success in the following ways:

  • Complete routine and emergency face-to-face recertification visits for hospice patients, including admissions, transfers, and extended stays.
  • Assess patient needs, document findings, and collaborate with physicians and the interdisciplinary care team to manage symptoms and support the Plan of Care.
  • Provide bedside palliative care, education, and physical and emotional support to patients and families during periods of crisis.
  • Triage patient needs based on acuity and help prevent avoidable hospitalizations by addressing care needs in the home.
  • Maintain accurate and timely patient records, clinical documentation, billing, and coding in accordance with CMS and regulatory guidelines.
  • Collaborate with the Medical Director, attending physicians, branch leadership, and interdisciplinary team to coordinate quality patient care.
  • Provide training and continuing education to clinical staff to support effective hospice and palliative care.
  • Assist in developing clinical practice guidelines and standards that promote high-quality palliative care.

What is Required?

  • Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations
  • Master’s degree in nursing and certification through a national Nurse Practitioner Licensing board
  • 1+ years of clinical nursing experience
  • Maintain active DEA certification if state allows prescription of controlled substances by APPs
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
  • 1+ year post-graduate clinical nursing experience in the hospice and palliative care field preferred
  • Prior experience in Family Medicine, Geriatrics and psychiatry preferred
