• Care nursing home patients with clear ownership and a treat-in-place approach.

What You’ll Do

• 
  Provide primary care to a steady patient panel
• 
  Lead on-site care planning and treat patients in place
• 
  Assess patients and build clear care plans
• 
  Manage chronic conditions, medications, and prior authorizations
• 
  Close HEDIS (quality measures) and STARs care gaps
• 
  Coordinate care with families and clinical teams

Must-Haves

• 
  Active Nurse Practitioner license in Ohio
• 
  Current national NP certification
• 
  Eligible to participate in Medicare
• 
  6+ months of Nurse Practitioner experience
• 
  Experience managing chronic conditions
• 
  Comfortable using clinical technology

Nice to Have

• 
  Nursing home care experience
• 
  Geriatric or palliative care experience
