The Position:
As a Nurse Practitioner at Snowline Health, you will be responsible for providing compassionate and comprehensive care to our patients and their families. Our Nurse Practitioner plays an important role in the care journey for our patients and families! As our Nurse Practitioner, you will collaborate with our Medical Directors and other care team disciplines to provide support to our care programs in all 3 counties that Snowline Health serves. Snowline's clinicians are experts in providing patients relief from the symptoms, pain, and anxiety while walking patients and caregivers through the difficult and often challenging healthcare systems. Snowline, established in 1979, is a nonprofit, community-based organization dedicated to serving our community by enhancing the lives of those with chronic, serious illness and patients nearing end-of-life. The care provided to patients and families is carried out by Snowline's Interdisciplinary team of professionals, including physicians, nurse practitioners, registered nurses, licensed vocational nurses, medical social workers, spiritual care providers, hospice aides, and volunteers.
Snowline's dedicated staff are experts in delivering end-of-life care, with many earning advanced certifications from the Hospice and Palliative Credentialing Center and the Hospice and Palliative Nurses Association. Snowline is a proud Employer Partner with the Hospice and Palliative Nurses Association and encourages our team to seek advancement and certification.
Hospice benefits are included in Medicare and Medi-Cal and most health insurance plans. The uninsured and palliative care recipients are also served at Snowline through funds from the proceeds of Snowline's thrift stores, a recycle and repurpose processing center enterprise, individual donations, fundraisers, and the United Way.
At Snowline, neither patient nor family will ever receive a bill for services.
We serve the western slope of El Dorado, Placer, and Sacramento County, including Sacramento, Folsom, Fair Oaks, Elk Grove, Roseville, Granite Bay, Fair Oaks, Carmichael, Lincoln, Rancho Cordova, and Rancho Murieta.
