Compensation: $105,000 - $140,000 Annual Salary Benefits: Medical, dental, vision, life insurance, long-term and short-term disability insurance, Malpractice Insurance, and 401K with a match program. PTO. CME Reimbursement.

We are looking for a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) for our Rehabilitation Facility in Rhode Island for bedside patient care. The provider serves as the primary medical contact for rehabilitation residents, partnering closely with the Medical Director, therapy teams, and interdisciplinary staff. This is a full-time, in-person position with predictable weekday hours and strong work-life balance.

Qualifications:

• Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Rhode Island, in good standing

• Active DEA registration

• 1+ years of clinical experience; SNF, rehabilitation, or primary care preferred

• Strong clinical documentation skills and familiarity with EMR systems

• Excellent communication skills with patients, families, and interdisciplinary teams

Responsibilities:

• Upon admission, examine residents and review their medical records to establish baseline assessments

• Conduct routine follow-up visits and manage chronic conditions during the rehabilitation period

• Prescribe and adjust medications including controlled substances per state regulations

• Coordinate care with PT, OT, and speech therapy teams

• Lead discharge planning for residents returning home or transitioning to other levels of care
