
Schedule
Part-Time: 3-5 hours per week with a weekly rotating on call responsibility.
The Nurse Practitioner (NP) provides on-site physical health services for individuals served in THRIVE’s NYS OASAS-licensed residential programs. The Nurse Practitioner will also be part of a rotating medical on-call coverage schedule which includes the three adult Part 820 Reintegration Community Residences and the youth Part 817 Residential Rehabilitation Services for Youth (RRSY) program. The NP plays a key role in integrating physical and behavioral health care, supporting recovery, and promoting wellness. This position works closely with nursing staff, counselors, case managers, and leadership to ensure high-quality, person-centered care.

Requirements
QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Required: Current New York State license and certification as a Nurse Practitioner (FNP, ANP, or other appropriate specialty). Current DEA registration, Medicare and Medicaid Credentialed.
Preferred: Experience in substance use disorder treatment, residential care, community health, or primary care. Familiarity with NYS OASAS regulations. Strong clinical judgment, communication skills, and ability to work collaboratively in a multidisciplinary environment.
CERTIFICATES, LICENSES, REGISTRATIONS
Required: Must maintain valid driver’s license and acceptable driving record. Current New York State license and certification as a Nurse Practitioner (FNP, ANP, or other appropriate specialty). Current DEA registration, Medicare and Medicaid Credentialed.

Transitional Living Services of Northern New York (TLS) is a private not-for-profit agency which was incorporated in 1977 to establish residential services for individuals in need of psychiatric, rehabilitative support who were unable to live independently in the community. TLS currently offers Adult Services, Children's Services, and has two Behavioral Health and Wellness Centers.
