Development and Leadership Opportunities in a Supportive, Multi-Disciplinary Team
About the Opportunity
Uniting NSW.ACT (Sydney Central) is launching an NP-led dementia care program across our Residential Aged Care Homes (RACHs) in the Central & Eastern Sydney PHN region.
As our Nurse Practitioner (Dementia Care), you will act as an advanced practice clinical leader embedded within care teams to improve assessment, prescribing (within scope), symptom management, and coordination with GPs and hospital outreach — reducing avoidable transfers and uplifting capability across our services.
We are seeking a dedicated Full-Time endorsed Nurse Practitioner, Nurse Practitioner Candidate (provisional), or experienced NP Generalist to join our Seniors Services Team for Sydney Central.
This position can be based in our Leichhardt regional office, with travel required across sites.
As a Nurse Practitioner, you will provide clinical services to residents across our 9 Residential Aged Care facilities.
This role is broad in scope and includes acute care episodes, chronic disease management, wound care, psychotropic stewardship, dementia care, deprescribing, and care coordination. You will work collaboratively with Residential Aged Care teams, GPs, and multidisciplinary stakeholders, focusing on best practice, capability building, and improving clinical outcomes for older people.
Key Responsibilities
Deliver comprehensive assessment and management for residents with dementia, using both non-pharmacological and pharmacological strategies (within scope and level of endorsement). Provide onsite clinical support and collaborate with GPs and care teams to ensure continuity of care. Coordinate case conferences and contribute to integrated care planning. Build capability through education, mentoring, and coaching of RNs, care staff, and families. Support dementia best practice and contribute to program reporting and outcomes.
About You
Current registration with AHPRA as:
An endorsed Nurse Practitioner,
Or a Nurse Practitioner Candidate (working toward endorsement),
Or a highly experienced NP Generalist with relevant advanced practice experience in aged care
Minimum 5 years nursing experience, including at least 12 months in aged care or a related field.
Demonstrated experience or strong interest in dementia care and managing complex behaviours in older people
Ability to work at an advanced practice level appropriate to your endorsement/candidacy, with support provided for development where required.
Skilled collaborator with primary care, allied health, and hospital services.
Strong communication skills with the ability to educate, lead, and influence practice.
Commitment to culturally safe care, particularly for Aboriginal and Torres Strait Islander peoples and multicultural communities.
Willingness to travel across sites and meet compliance requirements (vaccinations, police check, etc.).
