Job Category: NurseWe are seeking a dedicated and compassionate Nurse Practitioner (APRN) to provide high-quality primary and transitional care services to patients in Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), and Independent Living Facilities (ILFs).
This role focuses on delivering patient-centered care to a medically complex and often geriatric population, with an emphasis on improving outcomes, reducing hospitalizations, and enhancing quality of life.
As a member of the Transitional Care Physicians team, you will collaborate closely with facility staff, physicians, and interdisciplinary teams to ensure seamless, coordinated care.
Key Responsibilities
- Perform comprehensive patient assessments, including history and physical exams, in SNF, ALF, and ILF settings
- Diagnose and manage acute and chronic medical conditions
- Develop and implement individualized care plans
- Prescribe medications in accordance with Georgia regulations and clinical guidelines
- Coordinate care with facility staff, specialists, and families
- Review and follow up on labs, imaging, and diagnostic results in a timely manner
- Respond to clinical communications and urgent patient needs promptly
- Address nursing staff concerns within established response timeframes
- Document all patient encounters accurately within 48 hours
- Participate in interdisciplinary team meetings and facility rounds
- Meet productivity and visit expectations
- Participate in rotating on-call schedule, including holidays
Qualifications
Qualifications
- Active and unrestricted Georgia APRN license
- Board Eligible or Board Certified Nurse Practitioner
- Experience in SNF, ALF, ILF, geriatrics, hospice, or palliative care preferred
- Valid driver’s license and reliable transportation (travel between facilities required)
- Eligible for malpractice insurance coverage
- Strong clinical judgment and ability to work independently
- Excellent communication and collaboration skills
Work Environment
- Daily travel between Skilled Nursing, Assisted Living, and Independent Living facilities
- Regular on-site presence (typically 4–6 hours per day per facility)
- Collaborative, team-based care model
- Use of EMR and mobile communication tools
Physical Requirements
- Ability to stand, walk, and perform patient assessments throughout the day
- Frequent use of hands for clinical and documentation tasks
- Ability to lift up to 25 pounds occasionally
- Visual acuity for clinical evaluations and documentation
