Nurse Practitioner (NP) – Aging Adults

What You’ll Do
• Provide primary and preventative care to geriatric patients in home and community-based settings
• Manage chronic conditions with a focus on quality of life and functional outcomes
• Perform assessments, diagnoses, and treatment planning
• Collaborate with interdisciplinary teams, caregivers, and families
• Support transitions of care and reduce unnecessary hospitalizations
• Document visits and care plans in a timely and accurate manner

What We’re Looking For
• Licensed Nurse Practitioner (APRN) in Nebraska (or ability to obtain)
• Board certified (AANP or ANCC)
• Experience in geriatrics, primary care, hospice, or home health preferred
• Strong clinical judgment and ability to work independently
• Compassionate, relationship-focused approach to care
• Valid driver’s license and willingness to travel locally
