In this role, you will provide comprehensive healthcare services including assessment, diagnosis, treatment planning, medication management, and ongoing support for individuals with complex medical and behavioral health needs. You will work closely with multidisciplinary teams, collaborate across programs, and ensure high standards of care. Your work will support treatment within both primary care and substance use disorder settings.

Key Responsibilities:
· Provide holistic patient care through comprehensive physical and psychological assessments, detailed history-taking, and age-appropriate evaluations.
· Develop individualized treatment plans using evidence-based approaches, prescribing appropriate pharmacologic and non-pharmacologic interventions.
· Coordinate ongoing patient care by monitoring treatment effectiveness, adjusting interventions, and ensuring continuity across healthcare settings.
· Facilitate medication services within Substance Use Disorder programs, including mental status exams, symptom reviews, medication education, and side-effect screening.
· Collaborate with multidisciplinary team members to support patient transitions, make referrals, and contribute to care planning.
· Oversee team-based care coordination activities, including obtaining test results, communicating with external providers, and participating in daily huddles.
· Develop and approve standing orders in accordance with State law and organizational guidelines.
· Stay updated on best practices in addiction and mental health treatment to ensure high-quality clinical care.
· Support vulnerable populations through compassionate communication, education, and culturally sensitive care approaches.
Education, Experience, and/or Credential Qualifications:
· Graduate of an accredited school of nursing and accredited Nurse Practitioner program.
· Licensed as a RN in state of practice.
· Licensed as an APRN in state of practice.
· Two years of experience as a Nurse Practitioner.
Additional Qualifications:
· Current professional license within state of practice in good standing.
· Obtain a Federal DEA license.
· Current driver’s license, acceptable driving record and current auto insurance.
· Ability to relate well with patients and healthcare personnel.
· Social and cultural sensitivity appropriate for ethnically and economically diverse populations.
· Willingness to explore/obtain DATA 2000 Waiver.
· Applicant must be willing to become licensed in additional states as needed.
· Applicant must be willing and comfortable with providing telehealth services.
Physical Requirements
· Sedentary work involving up to 10 pounds of force occasionally and negligible force frequently to lift, carry, push, pull, or move objects.
· Frequent repetitive movements of hands, fingers, and arms for typing and writing.
· Mostly seated work with occasional walking or standing for brief periods.
