Full-Time Nurse Practitioner – Long-Term Care / Skilled Nursing
Baton Rouge, LA

We are seeking a full-time Nurse Practitioner to provide comprehensive medical management within long-term care and skilled nursing facilities in the Baton Rouge region.

Responsibilities

  • Provide primary and post-acute medical care in skilled nursing and long-term care facilities
  • Perform admission, regulatory, routine, and acute visits
  • Conduct comprehensive physical exams and manage chronic and acute conditions
  • Monthly order reviews and gradual dose reductions for psychotropic medications.
  • Order and interpret diagnostic tests, labs, and imaging
  • Prescribe and manage medications and individualized care plans
  • Collaborate with interdisciplinary facility teams to support quality outcomes
  • Ensure compliance with federal and Louisiana long-term care regulations
  • Identify changes in condition and intervene to prevent avoidable hospitalizations
  • Support goals-of-care discussions and advance care planning
  • Participate in after-hours or on-call rotation as needed

Qualifications

  • Active Nurse Practitioner license in Louisiana
  • 1 year experience as a Nurse Practitioner preferred
  • DEA license required
  • Experience in long-term care, skilled nursing, hospice, or post-acute care preferred
  • Strong critical thinking and clinical decision-making skills
  • Excellent time management and ability to work in a fast-paced environment
  • Ability to work independently while collaborating with facility staff
  • Strong documentation and communication skills

Preferred

  • Familiarity with Louisiana long-term care regulations
  • Experience in SNF/LTC medical management
  • Experience with PCC or other long-term care EMRs
  • Background in value-based or high-risk population management

St. Joseph Hospice, a compassionate and competent leader in the hospice industry. The team works with physicians to formulate a plan of care individually tailored to the needs of the patient. St. Joseph Hospice offers Routine Hospice Care, Respite Care, Crisis Care and General Inpatient Care.

Sage Rehabilitation Hospital and Outpatient Services offers an intense comprehensive program, utilizing a team of rehabilitation specialists; to guide you or your loved one along an individualized treatment plan designed to maximize functional independence.

Sage Specialty Hospital (LTAC) offers patients with complex medical conditions the continued intensive medical care they need when discharged from a traditional acute care hospital but need additional time to recover before they can go home or be admitted to a rehabilitation hospital or skilled nursing facility.

Capitol House provides a "home away from home"​ for those unable to remain living in their own residence. Offering rehab, respiratory services (including vent management), skilled nursing and more, the staff at Capitol House is qualified and compassionate.

Companion Home Services helps each client feel comfortable and confident in their homes. From private duty services to support with grooming and meal planning to help with errands or even just friendly companionship, we provide a number of services that add to the quality of life and helps our clients retain their sense of independence no matter what their situation.
