Job Description

A professional RN with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an Oncology Nurse Navigator provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.

Responsibilities

·The ONN acts as an advocate to assist the high-risk patient population in navigating throughout internal & external systems in order to facilitate the diagnosis and treatment process. Participates in a New patient visit and education in order to assist patients in understanding diagnosis, treatment options and resources available internal & external to the organization. Serves as liaison in planning care between clinic RN, patients, caregivers and multi-disciplinary health care providers with the goal being to reduce cancer morbidity and mortality by eliminating barriers to timely access to cancer care. Identifying strategies to eliminate barriers to care and coordinate among multi-disciplinary team to address. Facilitates transitions in care and communication (i.e. Hospice, survivorship, SNF, Home Care, etc.).

·Acts as a liaison between the physicians, health care staff, and the patient to ensure a successful episode of care. Follows up on patients after invasive procedures/surgeries/admissions/ED visits/chemotherapy treatments, etc.

·Communicates/Coordinates care among broader multi-disciplinary teams and patient's additional provider teams and referring physician to keep them informed of the status of a patients care/treatment from the start of their new patient visit note.

·Participates in review of quality metrics in an effort to maximize our outcomes, collects data and prepares various reports pertinent to the department, especially when pertaining to accreditations regionally & site specific. Assist with quality improvement activities.

·Coordinates patient paperwork needs, monitors and manages MediTech worklists, triages incoming critical results, and monitors for new symptom management needs. Assists with updating Care plans and Oncology history.

· Assists in policy and procedure development, revision, and implementation to maintain compliance with current state and national guidelines, standards and coding regulations

·Participates in staff education on an individual or group basis as needed; updates nurse manager on changes that could impact care delivery; maintains regulatory knowledge of specialty area at the federal, state, and local level. Works towards proficiency of Oncology Nursing Society ONN competencies. Engages in professional development and ongoing learning.

·Completes any other duties required or delegated to the Oncology Nurse Navigator. Assists with clinic through put and efficiency as necessary

· Attends and participates in a minimum of 50% department staff meetings and signs off on review of 100% department staff meeting minutes

· Completes all required annual education on or before the due date

Knowledge Skills and Abilities: Individuals must possess these knowledge, skills, and abilities or be able to explain and demonstrate that they can perform the primary functions of the job, with or without a reasonable accommodation, using some combination of skills and abilities and possess the necessary physical requirements, with or without a reasonable accommodation, to safely perform the essential functions of the job.

· Minimal physical effort required with occasional light lifting to a maximum of 25 pounds; stooping; and pick, pinch, type, or otherwise work primarily with fingers

· Ability to effectively manage considerable mental stress

· Ability to express or exchange ideas by means of the spoken word

· Ability to receive detailed information through oral communication

· Ability to communicate effectively with co-workers and convey information in a clear, concise and accurate manner

· Familiarity with computer and other business machines

Qualifications

Education: Nursing degree

Experience: previous experience in an oncology environment preferred; care coordination or navigation helpful. Minimum of two years of previous experience in clinical specialty.

Licenses/Certifications: Current NH RN, BLS, ONCC related oncology certification must be obtained within 12 months of hire.
