
The Clinical Nurse Navigator/Coordinator functions as a member of the multidisciplinary team who has clinical nursing expertise in an assigned area. They focus on the clinical and educational aspects of care, acting as a clinical resource, advocate, and educator. They are present in the clinic and serve as a vital member of the care team.
Essential Duties and Responsibilities
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned.
Assess factors of the patient’s past and present physical, psychosocial, and cultural baseline, health problems, and external support systems that may impact his/her ability to cope with the current health situation.
Provides excellent customer service by demonstrating ICARE values and positive behaviors towards patients, families/visitors, providers, other department staff, and co-workers.
Communicates problems and concerns to supervisors and provides input to support total quality improvement activities
Clinical Responsibilities (based on assignment):
Postpartum Follow-Up as assigned
Review delivery report and scheduling
Follow up patients that have recently delivered and ensure they have appointments scheduled
Follow up with hypertension patients and patients at high risk for maternal mood disorder to ensure proper follow-up
