The Registered Nurse Navigator SLPG acts as a liaison between the patient, patient’s family and caregivers and clinical care staff and helps navigate them through the treatment process by connecting them to resources and providing information to assist in making informed decisions

JOB DUTIES AND RESPONSIBILITIES:

  • Serves as a bridge between patients and clinical / medical staff
  • Guides patients, patient’s family and or caregivers through the healthcare process and assists in coordinating a plan of care which may include appointments, transportation, education, and provision of and / or access to support services.
  • Advocates on behalf of the patient
  • Ensures patients understand their disease process and treatment options
  • Acts as a liaison between clinical / medical staff and patients during their treatments.
  • Manages the cross-continuum care of multidisciplinary patients to provide high quality care in an effective and efficient manner with close attention to service excellence. Provides multidisciplinary team with appropriate information and patient summaries.
  • Serves as a consistent contact for patient, patient’s family, and caregivers throughout the process of diagnosis and treatment. Empowers patient/family to participate in the plan of care, documents compliance and concerns, monitors patient progress, consults with physicians and other members of the health care team at agreed upon intervals.
  • Collaborates with clinical leadership per specific service line.
  • Interacts with administration, manages and facilitates program improvement, keeps team up to date with program and monitors progress in achieving desired outcomes.
  • Identifies patient and/or provider needs and ensures delivery of quality services to patients.
  • Other related duties as assigned.

PHYSICAL AND SENSORY REQUIREMENTS:

Sit up to 7 hours per day; 3 hours at a time. Stand for up to 3 hours per day; 2 hours at a time. Walk 1 hour per day; 10 minutes at a time. Frequently lifting, carrying and pushing objects up to 10 pounds. Rarely lifting, carrying and pushing objects up to 75 pounds. Frequently stooping and bending. Frequently reaching above shoulder level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, high and low frequencies. Must be able to see as it relates to general near, far, color and peripheral vision. Must be able to speak as it relates to normal conversation and projection as it relates to lectures and presentations.

EDUCATION:

Registered Nurse with current license to practice in the State of Pennsylvania or seeking Pennsylvania licensure through reciprocity. NJ RN licensure required for New Jersey locations required.

Bachelor’s degree preferred.

Specialty-specific RN certifications may be required based on service line

TRAINING AND EXPERIENCE:

Three to five years specialty specific experience preferred.
