The Nurse Manager holds a critical administrative position responsible for coordinating and monitoring primary care service delivery, quality assurance, patient utilization, staff coverage, medical record completion, and releases to DHS partners incident to patient HIPAA releases. Conflict resolution for staff and patient issues and medical/administrative interface functions for the Care for The Homeless Health Centers are essential. The position requires broad health care and management expertise in a multi-service, shelter-based health center.

This employee will work as part of the PCMH team with a Trauma Informed Care approach to carry out the following essential duties and responsibilities.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide administrative oversight for support and clinical staff in a shelter-based health center facility.
  • Act as the administrative liaison with the shelter Administrative and Case Management staff to ensure resident and patient workflows consistent with Department of Homeless Services’ safety protocols.
  • Coordinate work schedules of all health center staff;
  • Ensure Article 28 compliance with all applicable regulations, including (but not limited to):
  • Ensuring that medical services are provided at the center only by members of the medical staff of the center.
  • Ensuring that all equipment is maintained in safe and working order.
  • Ensuring the site complies with all CFH established policies and procedures addressing:
  • The provision of service(s) without regard to age, race, color, sexual orientation, marital status, religion, sex, national origin, or sponsor.
  • Provisions for communicating with patients who exhibit a communication barrier
  • Ensuring prompt follow-up action on patients with abnormal test results or physical findings.
  • Coordination with Health Information Management (HIM) to assure the prompt transfer of a copy of the pertinent parts of the medical record and other information, with the patient's written consent, when the patient is referred to another health-care facility or healthcare practitioner
  • Effective communication and coordination with all members of the health care team to ensure implementation of effective care coordination services for all patients.
  • Ensuring that are staff prepared to care for emergencies, and equipment is maintained in working condition
  • Provide day-to-day supervision and both clinical and administrative direction to all support staff within the center.
  • Provide appropriate initial and ongoing staff training for all new policies and operational procedures.
  • Ensure the timely completion of the all chart documentation by all staff at the health center.
  • Assure site participation in all staff meetings, medical staff meetings, required trainings.
  • Prepare reports and other documentation requested by regulatory agencies, partner organizations, and CFH management.
  • Review all weekly and monthly reports submitted to DHS by Care Coordination staff.
  • Acts as the official CFH liaison to DHS for the program sites.
  • Facilitate coordination of the telemedicine and virtual visit access model
  • Provide education and supervision for medical assistants, care coordinators and patient service representative staff including site specific schedules, goals, feedback and annual evaluations
  • Work collaboratively with Outreach and Population Health staff to effectively coordinate engagement events in partnership with the primary care medical staff
  • Collaborate with all members of the health team to facilitate the highest quality of services.
  • Coordinate the development, implementation, and evaluation of the nursing protocol for all clinic patients/clients.
  • Identify components of the nursing protocol that may be delegated to allied nursing personnel and provide teaching and guidance as necessary.
  • Supervise care coordinator workflow for all referrals and follow-up
  • Review and analyze trends pertinent to the delivery of care services at site (medical follow up, no-show rate, linkage to care, etc.) and implement outcome-based solutions.
  • Assure that a health center inventory system (medical equipment) that is effective and efficient is maintained.
  • Prioritize and designate assignments and responsibilities for staff to ensure optimum patient access and experience for the patients served.
  • Participate in daily huddle and assure all staff participation.
  • Assure use of the pre-visit planning tool is implemented at the site as well as the end of day report
  • Ensure daily reconciliation of the shelter roster is performed with the appointments scheduled to open appointments if shelter residents are no longer at the facility.
  • Review the daily, weekly, and monthly site reports to Department of Homeless Services (DHS) with the Care Coordinators and Administration.
  • Participate in the monthly CFH- DHS meeting to review performance to goal
  • Maintain professional licensure requirements through participation in continuing professional education and other appropriate learning experiences.
  • Perform other related duties, as assigned.

Qualifications

JOB QUALIFICATIONS:

  • Master’s degree in clinical or human services field from an accredited professional school.
  • Clinical Training (RN). Five years of direct patient/ client care
  • Three years of supervisory/ management experience working in an ambulatory health care facility.

Experience:

  • 3-5 years in senior management /and field experience in the development and operation of a diversified, decentralized health care facility.
  • Commitment to vulnerable and disadvantaged populations.
  • Flexible for hours needed and able to work in a nontraditional setting (shelter).

Skill Sets:

  • Refined organizational skills and leadership qualities, excellent communication skills
  • Knowledge of health care planning and administration within an ambulatory health setting serving a multi-problem clientele.
  • Ability to supervise a diverse staff of professionals and paraprofessionals in a multi-service setting.
  • Ability to represent the organization in community forums and educational seminars.
  • Excellent written and verbal communication skills; conflict resolution skills, demonstrated commitment to staff and personal continuing education and training to improve the ability to deliver superior service for the population served by CFH.
  • Proficient computer skills. Usage of EMRs.
  • Ability to use email and Teams, and texting as modalities of communication daily.
