Description
Under the direction of the Director of Nursing, oversees residents and staff for half of the building. Is a resource for the charge nurses and CNAs/CMAs .
1. Originates initial care plan upon admission, and follows up when quarterly assessments are due.
2. Double check the nursing check list to ensure completion and compliance. Assists nurses with education if check list is not completely completed.
3. Assists the MDS coordinator with information for quarterly assessments
4. Updates the Matrix three times per week.
5. Assists in any area on the unit as needed.
6. Conferences with families as needed.
7.Assist clinic nurse and cover clinic in clinic nurse absence.
8. Draw lab and help with tele-med appointments.
9. Dietary consults. Obtain changes requested by dietitian, follow through with any requested changes.
10. Assist with new admissions.
11. Training of newly hired nurses.
12. Attend Medicare/risk meetings and follow up on any requested changes.
13. Staff education as needed.
14. Spot checks on medication carts.
15. Obtain orders/changes made during care plan meeting.
16. Make sure information from MDS and Infection Control is given to staff and thy understand the request.
17. Assist Therapy team wih education and updates.
18. On call one weekend a month.
19. All other duties as assigned.
QualificationsQualificationsMust have a valid Kansas LPN or RN license. Knowledge of Long Term Care. Good time management. Ability to work well with people. Attention to detail. Strong resident advocate. Good communication skills.
