About the program
Casa Esperanza’s Conexiones program is a 32-bed facility that provides Individualized Treatment Services Program (ITS) which delivers integrated acute treatment services and withdrawal management and clinical stabilization services (CSS) to adults struggling with co-occurring substance use and mental health treatment needs. At Casa Esperanza’s Conexiones ITS program, located at Tewksbury Hospital, we provide in-patient addictions and mental health treatment in a physically and psychologically safe environment, delivered by medical, clinical, peer support, and paraprofessional staff under the direction of a Medical Director. We serve adults in a structured, yet individualized treatment environment focused on building the foundation for a sustainable recovery, and developing a healthy, productive and meaningful lifestyle. The program is staffed by highly skilled professionals who work with each patient to develop an individualized treatment plan, deliver evidence-based individual and group treatment, and develop an appropriate aftercare plan and establish placement in community-based care.
About the role
We are currently seeking an experienced Nurse Manager to provide leadership and supervision for all Nursing, Unit Management, and Recovery Specialist Staff at our co-occurring enhanced ITS program. Through Casa’s ITS Model (individualized and integrated care model), the Nurse Manager will work collaboratively with our Medical Director, NP Medical Director Designee (FNP), and our PMHNP, to lead our nursing team, provide the highest quality patient care, implement doctor’s orders, monitor specified medication/refills, monitor and record vital signs as needed, and maintain thorough and timely medical/nursing records. The Nurse Manager will observe patients and provide timely care and documentation to include patient progress in treatment, care coordination, and care management, advising Case Managers regarding appropriate placement in community.

Our mission is to empower individuals and families to recover from substance use disorder, trauma, mental illness, and other chronic medical conditions; overcome homelessness; and achieve health and wellness through comprehensive, integrated care.
Our services:
Residential Recovery: Casa Esperanza's Men’s Program serves 29 adult men and Latinas y Niños serves 20 adult women and can also house up to 12 of children age ten or younger so clients in recovery may maintain or regain custody while still in treatment. Residential services usually last six to twelve months and offer evidence-based treatment methodologies as well as assisting clients with setting long-term goals for maintaining their recovery as they transition to independent living.
Structured Outpatient Addictions Program (SOAP): Familias Unidas Outpatient Services is a licensed, CARF-accredited clinic that provides client-centered, integrated services to support the complex, long-term needs of people with co-occurring disorders. Familias is co-located with primary care services and is the only Spanish-language SOAP in Greater Boston.
Supportive Housing: Nueva Vida provides15 single-occupancy units, Dunmore Place & 300 Eustis Street provides eight family units, and Nueva Esperanza provides 14 individual studio units for a total of 37 units of permanent safe, sober housing for individuals and families, with on-site support for maintaining recovery and building independent living skills.
Clinical Stabilization: Conexiones is a structured 24-hour inpatient clinical stabilization program for individuals early in their recovery which includes nursing care, individual and group therapy, psycho-educational groups, case management, health education, and aftercare planning.
