The Licensed Practical Nurse (LPN) provides essential nursing support in a transitional housing program serving clients with mental health and stabilization needs. Working collaboratively with case managers, social workers, psychiatrists, and program staff, the LPN ensures client well-being through assessment, medication management, crisis response, and care coordination. Responsibilities adapt to program needs and prioritize client safety, dignity, and continuity of care.

Primary Job Responsibilities/Duties

TheNurse (LicensedPractical)isresponsible for, butnotlimited to:

  • Conduct patient assessments during routine interactions,identifyingphysical, behavioral, and psychological needs.
  • Build trusting relationships with clients by actively listening, interpreting concerns, and responding with empathy.
  • Verify client identity and ensureaccuratemedication administration in compliance with program protocols.
  • Respond calmly and effectively to distressed or escalated clients using de-escalation techniques.
  • Identifyrisks of self-harm or harm to others andinitiateappropriate safetyinterventions.
  • Maintain secure medication storage units andaccurateinventory records.
  • Prepare and update client records, including progress notes, care plans, and discharge summaries.
  • Develop individualized care plans in consultation with psychiatrists, case managers, and social services staff.
  • Coordinate client transitions out of the shelter, including after-care follow-up with case managers.
  • Participate in team meetings, caseconferences, and external consultations asrequired.
  • Foster respectful, collaborative relationships with colleagues to support high-quality, integrated care.
  • Continuously seek innovative approaches to improve service delivery and client outcomes.
  • Communicate clearly and professionally in written and verbal forms, adhering to ethical, legal, and documentation standards.
  • Manage time, resources, and competing priorities effectively in a fast-paced environment.
  • Maintain clinicalexpertisein acute mental health care, including suicide risk assessment and crisis management.

Physical Requirements

  • Ability to walk, stand, kneel, crouch, and bend over.
  • Ability to lift, reach, and handle objects weighing up to 25 pounds.
  • Ability to sit at a desk and work on a computer for prolonged periods.
  • Ability to travel todifferent locationswithin New York City, as needed.

Work Environment / Schedule Requirements

  • This is a full-time, exempt position. While standard business hours are Monday through Friday, 9:00 AM to 5:00 PM, exempt employees are expected toworkthe hours necessary to effectively fulfill their job responsibilities, and based on organizational needs, deadlines, or events.
  • Staff must continue working past their shift unless relieved by another staff member or authorized to leave by a supervisor.

Qualifications

  • Active New York State LPN license.
  • Minimum five (5) years of nursing experience in mental health settings.
  • Bilingual in English and Spanish or English and French.
  • Mental Health Nursing Certification is a plus
  • Experience in homeless services, transitional housing, or crisis intervention is preferred.
  • Must be able to read, speak, write, and understand English for administrative purposes.
  • Must pass drug screening to be appointed.
  • This position may be subject to a series of investigations before and after appointment.
