Job Title: Nurse Educator

Responsibilities:
Develop and implement individualized education plans for residents and their families, focusing on disease prevention, symptom management, and overall health promotion
Provide education and support to residents on medication management, self-care, and disease management
Collaborate with interdisciplinary team members to develop and implement evidence-based interventions and treatment plans
Conduct patient assessments and evaluations to identify learning needs and preferences
Design and deliver educational programs and workshops to staff, residents, and families on various topics related to health and wellness
Monitor and evaluate the effectiveness of education programs and make recommendations for improvement
Stay up-to-date with industry trends and best practices in patient education and continues to advance knowledge and skills
Requirements:
Current licensure as a Registered Nurse in the state of Pennsylvania
Strong communication and interpersonal skills
Ability to work effectively with diverse populations, including residents, families, and healthcare professionals
Strong organizational and time management skills
