University of Iowa Health Care Department of Nursing is seeking a Nurse Clinician-Discharge Coordinator (PICU) to perform specific tasks to assist members of the Nursing team in providing patient education and discharge planning under the supervision of the Director of Pediatric Care Coordination Division.Position Summary:The Nurse Clinician-Discharge Coordinator for the Pediatric Intensive Care Unit at University of Iowa Stead Family Children’s hospital functions as a clinical expert and care coordinator for pediatric patients with special and complex healthcare needs. The discharge coordinator manages care for the identified specific ICU patient population including IMCU (Intermediate Care Unit) and follows these patients through the continuum of care. The discharge nurse optimizes utilization of resources and service delivery including promoting patient throughput. They provide ongoing support and expertise through comprehensive assessments, discharge care planning, plan implementation and overall evaluation of patient’s needs for the next level of care. They work closely with the interdisciplinary team to enhance communication through daily bed huddles and rounding between team members, patients and their families.
Responsibilities will include, but are not limited to: Discharge process: ensuring discharge orders are complete, reviewing discharge summary and AVS for appropriate HCI, provide teach back with patients/families, monitoring prescription fill time to pharmacy, assist with obtaining necessary supplies upon discharge if needed, ensure follow-up appointments are scheduled when possible and are accurate on AVS, and provide patients the clinic and emergency contact information on discharge.Lead daily unit bed huddles in coordination with the multidisciplinary team to manage care and review patients’ treatment plan with a focus on discharge planning. (why are they here, what are the goals to discharge and what needs to be done to achieve discharge).Collaborate with unit social work, patients and families to discuss overview of discharge plan, equipment needs and education. Collaborates with multidisciplinary team to facilitate timely discharge and escalate length of stay barriers.
Percent of Time: 100%
Schedule: 40 hours per week, scheduled between Monday-Friday (0730-1600). No weekends or holidays

Required Qualifications: A Baccalaureate degree in Nursing Reasonable (within the last 1-3 years) Pediatric ICU nursing experience is required.Demonstrated expertise in specific clinical areas of practice required.Excellent written and verbal communication skills are required.Demonstrate excellent organizational skills, problem solving and critical thinking Current license to practice nursing.
