Nurse Clinical Manager – Stuart, Florida

Activa Home Health, an AccordCare Company, is seeking an experienced RN Clinical Manager to lead clinical operations in our Stuart office. This is a high-impact leadership role for a Medicare home health professional with strong OASIS, case management, compliance, and team support experience.

This primarily in-office position includes clinical oversight, admissions coordination, staff mentorship, quality assurance, and field support. The ideal candidate is customer service–driven, highly organized, tech-savvy, and confident managing fast-paced home health operations while ensuring compliance with Medicare and Florida home health regulations.

Qualifications:

• Current Florida RN license in good standing
• Minimum 1 year Medicare home health case management experience
• Strong knowledge of OASIS and home health compliance standards
• EMR experience; Kinnser/WellSky preferred
• Excellent organizational, communication, leadership, and customer service skills
• Ability to multitask and support both clinical and operational workflows
