Job Description

The Sound Capitol Hill clinic has received a four-year Certified Community Behavioral Health Center (CCBHC) grant through SAMHSA (Substance Abuse and Mental Health Services Administration). Part of this grant includes implementation of comprehensive healthcare integration to ensure that clients with behavioral health disorders are screened for physical health conditions, referred for appropriate medical care and collaboratively supported in their ongoing health needs. In response to this, we are welcoming a Nurse Care Coordinator and Trainer in Seattle, WA!

The Nurse Care Coordinator and Trainer supports integrated, whole-person care for individuals receiving services in a Certified Community Behavioral Health Clinic (CCBHC). The Nurse Care Coordinator and Trainer provides clinical and operational leadership for nursing-led care coordination and primary care screening activities within the CCBHC.

This role sets standards, guides workflows, and supports consistency and quality of nursing practice by providing direction, guidance, and subject-matter expertise to nursing staff and multidisciplinary teams across CCBHC sites. Working as part of a multidisciplinary team, the Nurse Care Coordinator and Trainer provides clinical nursing services, care coordination, and health education to individuals with serious mental illness (SMI), substance use disorders (SUD), and co-occurring chronic medical conditions.

This role emphasizes care coordination, population health management, and collaboration, rather than episodic medical treatment, and supports CCBHC requirements for comprehensive, person-centered, and coordinated care. The Nurse Care Coordinator lead will provide nursing assessment and screenings, clinical education, care planning, and support to clients at Sound clinics throughout King County. They will also support the nursing team from the other clinical sites at Sound as the entire organization transitions to becoming a CCBHC.

Location: Cap Hill Clinic (1600 E Olive St Seattle, Washington 98122 United States) with flexibility to travel throughout King County.

Schedule: Monday-Friday 8:30 AM- 5:00 PM

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Implement primary care screening & monitoring workflows (e.g., complete vital signs, engage clients in tobacco use & cessation counseling, support behavioral changes related to BMI/weight management, monitor chronic health conditions, hypertension & diabetes monitoring) and documents associated quality measures for CCBHC reporting.
  • Coordinate physical and behavioral health care across internal programs and external providers (primary care, specialty care, hospitals, detox, OTPs, etc.)
  • Develop and provide education about chronic health conditions and health-related behaviors for clients, clinical teams, and outpatient clinics
  • Educate clients on health management, medication adherence, and wellness
  • Support clients and facilitate their engagement in primary care services and provide care planning for clients and ongoing coordination of care with pharmacy, primary and other specialists
  • Lead closedloop referrals to primary/specialty care: initiate, track, confirm appointment completion, and integrate results into the client’s person centered treatment plan.
  • Partner with leadership to operationalize care coordination agreements/MOUs with FQHCs/PCPs, hospitals/EDs, OTPs, and social service agencies; maintain workflows for information exchange and warm handoffs.
  • Complete outreach and collaborate the clinical teams to encourage client engagement in nursing services.
  • Provide medical support to medical providers.
  • Obtain vital signs, urine drug screens and basic labs (including phlebotomy).
  • Comply with documenting and reporting requirements.
  • Participate in stabilization and maintenance phases of medication-assisted treatment of clients. Provide relapse prevention support to clients.
  • Collaborate with the CCBHC Director, Project Director and Program Evaluator to ensure the successful implementation of care delivery at CCBHC sites. Ensure that grant requirements are met regarding data collection for the screening and monitoring of chronic health conditions per CCBHC requirements.
  • Assist with implementation of the four-year SAMHSA CCBHC grant including collaborating with internal CCBHC transformation team.
  • Other duties as assigned at the discretion of management.

Qualifications

EDUCATION and/or EXPERIENCE

  • Minimum of three years of nursing experience, preferably in behavioral health and / or primary care settings.
  • At least three years of experience with case management or care coordination preferred.
  • Experience working with individuals with mental health and/or substance use disorders
  • Knowledge of integrated care models and recovery-oriented practices
  • Experience writing and implementing policies, procedures and educational interventions.
  • Strong documentation, communication, and collaboration skills

KNOWLEDGE, SKILLS & ABILITIES

  • Competency at basic nursing skills such as venipuncture, IM injections, vital sign monitoring.

  • Familiarity with community resources for behavioral health, substance use disorders and primary care.
  • Strong internal motivation and work ethic. Sound judgment.
  • Promptness and reliability in communication, attendance, work tasks and deadlines.
  • Active listening techniques that convey respect, professionalism, engagement, positivity, and empathy.
  • Knowledge related to adult education and learning styles

CERTIFICATES, LICENSES & REGISTRATIONS

  • Active Washington State RN license required.
  • Current First Aid and CPR certification required.
  • Must have a Washington State driver's license and an insurable driving record.
  • Must pass a criminal background check.

PHYSICAL DEMANDSTo successfully perform the essential duties of this position, an individual must be able to sit at a desk for six or more hours a day, and use office equipment, including phones and computer keyboards, for six or more hours per day.
