Reporting to the Primary Health Care Manager, the Primary Health Care Nurse is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The Primary Health Care Nurse promotes quality health care outcomes working within established standards for case management practice. The Primary Health Care Nurse initiates and/or participates in community development initiatives and participates in the evaluation of Primary Health Care initiatives within the site.

Human Resources Exemption: No

Experience

  • 2 years nursing experience in a community based health care setting preferred
  • Experience in teaching adults, facilitation skills, and leading group activities
  • Experience in case management is an asset

Education

  • Bachelor Degree or Diploma in Nursing or Psychiatric Nursing

Licenses

  • Basic Life Support Certificate (BLS)

  • Demonstrated clinical competency

  • Knowledge and application of quality improvement principles, tools, and outcomes
  • Knowledge of motivational interviewing
  • Knowledge in conflict resolution techniques
  • Ability to work under pressure and establish priorities
  • Demonstrates ability to accept and implement change
  • Knowledge of the impact of social/economic/cultural issues on chronic disease
  • Ability to apply evidence-based decision making
  • Systems thinking approach to health care delivery

  • Registered/eligible to be registered with RPNAS (Reg Psych Nurses Assoc of Sask)
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
