Reporting to a Continuing Care Saskatoon Urban Program Services Manager, the Supportive Care Projects Coordinator (SCPC) is responsible for mentoring and leading quality improvement and risk management in assigned special care homes within the Continuing Care Saskatoon Urban portfolio. This encompasses planning, design, coordination, education, implementation and evaluation of projects and initiatives as identified by the Department and the SHA. The SCPC, collaborates with other Departments and areas within the Saskatchewan Health Authority to improve and standardize practice in long term care through involvement in work at the provincial level as a working member or lead/co-lead. Through this work, the SCPC ensures best practice, Resident and Family Centred Care, and alignment of practices to Accreditation Canada Standards, the Program Guidelines for Special Care Homes, and any other applicable required stipulations or law.

Human Resources Exemption: No

Experience

  • One (1) year leadership experience (i.e. coordinator) or management experience
  • Minimum of two (2) years relevant long term care nursing experience
  • Experience with interRAI LTCF is required

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Valid Class 5 driver's license

  • Commitment to service excellence, in quality improvement and risk management
  • Advocacy, negotiation, crisis management and conflict resolution
  • Demonstrated ability to direct and facilitate change
  • Knowledge of research methods involving collecting data, analysis and praxis
  • Demonstrates effective verbal and written communication skills.
  • Ability to establish/maintain effective working internal/external relationships
  • Ability to work independently and as a member of a multi-disciplinary team
