Job Description:

Reporting to a Manager in Primary Health Care, in the North Network, the Chronic Disease Management Diabetes Nurse Educator responsibilities include; coordinating education for clients living with diabetes, providing best practice in diabetes management as part of a multidisciplinary team, providing individual inpatient and outpatient education for clients, facilitating group classes in the outpatient setting, promoting self-management education and participating in departmental activities such as program development, planning, quality improvement and research.

Experience

• 3 yrs clinical experience with patients and families living with diabetes.
• 1 yr experience in adult education/group facilitation related to diabetes management
• 1 yr experience in program development, needs assessment, planning, program design, implementation, and evaluation
• teaching/counselling, facilitation, and leading group activities is required.
• Experience working with complex patients and families in an outpatient setting.

Education

• Bachelor of Science in Nursing (BScN)

Licenses

• Valid Class 5 driver's license

Other Education and Training

• Master of Nursing, Adult or Continuing Education is an asset
• Basic Life Support Certificate (BLS)

Other Information

• Current certification as a Diabetes Educator (DES) is required within 2 years of employment and Membership with the Diabetes Education section is an asset.
• The Saskatchewan Health Authority (SHA) requires a criminal record check (CRC) from every potential SHA team member.
• Master’s Degree in nursing, adult or continuing education is an asset.
• The CRC must be dated within the past six (6) months and be satisfactory to the SHA. A vulnerable sector check (VSC) is required for individuals providing direct care over vulnerable adults, and children under 18.
