REPORTING TO THE CHRONIC DISEASE MANAGEMENT MANAGER - RESPIRATORY PROGRAMS, THE RESPONSIBILITIES OF THE CHRONIC DISEASE MANAGEMENT NURSE EDUCATOR WILL INCLUDE: PRIMARY RESPONSIBILITY FOR CHRONIC DISEASE MANAGEMENT PROGRAMS IN THE AREAS OF RESPIRATORY DISEASES - ASTHMA, ASSISTING WITH DEVELOPMENT/CO-ORDINATION/CONDUCT OF INPATIENT AND OUTPATIENT PROGRAMS. UTILIZE ESTABLISHED CRITERIA TO ASSESS AND PLAN EACH CLIENT'S SELF-MANAGEMENT CARE NEEDS, PROVIDING APPROPRIATE INTERVENTION TO ENSURE TIMELY ACCESS TO CARE. PROVIDE EFFICIENT AND EFFECTIVE INDIVIDUAL SELF-MANAGEMENT EDUCATION/COUNSELING FOR PATIENTS/FAMILIES/CAREGIVERS IN ALL ASPECT OF RESPIRATORY DISEASE MANAGEMENT CARE AND TREATMENT. THIS WILL INCLUDE LIAISON WITH FAMILIES, PHYSICIANS, DIETITIANS, STAFF NURSES, AND COMMUNITY AGENCIES. COORDINATE AND FACILITATE GROUP EDUCATION CLASSES FOR OUTPATIENTS IN A VARIETY OF SETTINGS. PROMOTE CONTINUING HEALTH PROFESSIONAL INVOLVEMENT IN PATIENT EDUCATION THROUGH EFFECTIVE COMMUNICATION, DEVELOPMENT OF RESOURCE MATERIALS AND PROVISION OF IN-SERVICE EDUCATION. PARTICIPATE IN DEPARTMENTAL/RESEARCH ACTIVITIES (PROGRAM DEVELOPMENT, PLANNING, QUALITY IMPROVEMENT, AND EVALUATION). IMPLEMENT PROGRAM/RESOURCE DEVELOPMENT RELATED TO NEEDS ASSESSMENT, PLANNING, PROGRAM DESIGN, IMPLEMENTATION AND EVALUATION.

Human Resources Exemption: No

Experience

  • Three (3) years nursing experience with patients/families with chronic disease
  • One (1) year experience in program/resource development
  • One (1) year experience in adult education

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Basic Life Support Certificate (BLS)
  • Basic Life Support Instructor Certificate (BLS)
