The Chronic Disease Management (CDM) Nurse Educator integrates in-depth clinical expertise and experience, knowledge of nursing theory and the principles of teaching and learning, in the provision of chronic disease education, and quality care for clients, families, health professionals and students. The CDM nurse educator is responsible for assessing learning needs, developing, coordinating, implementing and evaluating self-management educational/counseling activities for clients, families, health professionals and students in inflammatory bowel disease. The CDM nurse educator collaborates with the multidisciplinary team members, inpatient/outpatient/hospital departments/community agencies to establish partnerships, initiating referrals to community services as required. The CDM nurse educator uses a consultative approach, implementing accepted standards and guidelines and current research findings to advocate for continuity and coordination of care. Teaching can occur in inpatient, outpatient, specialty clinics, physician clinics, primary health centers or community settings, with individuals or groups.

Human Resources Exemption: No

Experience

  • A minimum of 1 year experience in patient/family/professional education
  • Three (3) years nursing experience with patients/families with chronic disease
  • One (1) year clinical experience in the specific department/program/area

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Basic Life Support Certificate (BLS)
