The CDM Clinical Nurse Educator reports to the Manager Clinical Integration and works with an interdisciplinary team with a chronic disease prevention and management focus. The CDM Clinical Nurse Educator integrates in-depth clinical expertise, experience, and knowledge with the principles of teaching and learning, in the provision of chronic disease self-care management for clients, families, health professionals and students.

Human Resources Exemption: No

Experience

  • 3 years clinical experience with patients/families with chronic disease

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Valid Class 5 driver's license
  • Certified Diabetes Educator (CDE)

Other Education and Training

  • Nursing diploma is acceptable
  • Basic Life Support Certificate (BLS)

  • Demonstrates word processing skills using Microsoft Office programs

  • Ability to competently use computers and software applications
  • Demonstrates ability in teaching, facilitation and group activities
