Job Description: Reporting to the Unit Manager, the Resident Care Coordinator is responsible for ensuring the highest standard of care and service is provided to residents/veterans on a given unit. As an experienced practitioner, the resident care coordinator acts in a leadership role to provide specialized clinical expertise, coordination of resident care, and support and teaching to the resident care team (including residents and residents' families) within a collaborative environment. The routine procedures are generally predetermined but critical thinking and independent judgment is an integral part of the position.

Experience

  • Minimum of three (3) years relevant long term care nursing experience

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Basic Life Support Certificate (BLS)

Other Education and Training

  • Nursing diploma is acceptable
  • Psychiatric Nursing diploma is acceptable
  • Bachelor of Psychiatric Nursing is acceptable

Knowledge and Abilities

  • Demonstrates effective verbal and written communication skills.
  • Facilitates the implementation/evaluation/design for client centered care model

  • Knowledge of mental health needs of adults & families, & clinical interventions
  • Work as a team member, can establish & maintain interpersonal relationships
