Position Summary
The Palliative Care Nurse Practitioner (NP) provides advanced clinical care to patients with serious and life-limiting illnesses. This role is responsible for conducting hospice face-to-face encounters, providing symptom management, and supporting patients and families receiving hospice and palliative care services in private homes, assisted living communities, and skilled nursing facilities.
The NP works collaboratively with the interdisciplinary team—including physicians, nurses, social workers, and chaplains—to ensure compassionate, patient-centered care that aligns with hospice philosophy and regulatory requirements.
Key Responsibilities
Patient Care & Clinical Management
- Conduct hospice face-to-face visits to determine continued hospice eligibility in accordance with CMS guidelines.
- Perform comprehensive patient assessments, including physical exam, symptom evaluation, and review of disease progression.
- Manage complex symptom control including pain, dyspnea, nausea, anxiety, agitation, and other end-of-life symptoms.
- Develop and implement individualized care plans in collaboration with the hospice interdisciplinary team.
- Provide palliative care consultations and support goals-of-care discussions with patients and families.
- Order and interpret diagnostic tests, labs, and medications as appropriate.
- Prescribe treatments and medications in accordance with state regulations and hospice protocols.
Hospice Compliance & Documentation
- Complete timely and accurate documentation in the electronic medical record.
- Ensure all face-to-face encounter documentation meets CMS hospice eligibility requirements.
- Participate in interdisciplinary team (IDT) meetings to discuss patient status and care planning.
- Maintain compliance with Medicare hospice regulations and agency policies.
Collaboration & Communication
- Work closely with hospice physicians, nurses, and care teams to coordinate care.
- Communicate with facility staff, caregivers, and family members regarding patient condition and care needs.
- Provide education to patients, families, and staff on disease progression, symptom management, and end-of-life care.
- Support transitions of care between home, assisted living, and skilled nursing facilities.

Qualifications
- Current Nurse Practitioner license in the state of practice.
- Board certification as a Nurse Practitioner (FNP, AGNP, or ANP).
- DEA license (or ability to obtain).
- Minimum 1–2 years of clinical experience as a Nurse Practitioner.
- Experience in hospice, palliative care, geriatrics, or home health strongly preferred.
- Strong clinical assessment and symptom management skills.
- Ability to travel to patient homes and care facilities.
Preferred
- Hospice and Palliative Care certification (CHPN or ACHPN).
- Experience performing hospice face-to-face encounters.
- Experience working within an interdisciplinary hospice team.
