Responsibilities

The Northside Connect Nurse, is a compassionate and highly competent liaison who serves as a core clinical resource within the Connect Call Center.

This nurse serves as a critical access point for Northside Hospital clientele by ascertaining their needs and seamlessly connecting callers to appropriate hospital providers, clinics and resources. Utilizing strong clinical judgement and compassionate communication, the nurse ensures patients receive timely guidance and access to care that aligns with their health care priorities.

With strong clinical assessment and advanced technical skills, this team manages over 10,000 monthly calls and serves as the Frontline for key interdisciplinary teams within the organization. The PR Nurse demonstrates exceptional critical thinking, active listening and empathy during the call ensuring each patient feels heard, supported and safely directed to the care they need.

In addition, this nurse plays a key role in post-discharge care coordination. Through structured follow-up calls, the nurse evaluates patient recovery progress, identifies early signs of complications, reinforces discharge instructions, and removes barriers that could lead to hospital readmission. By delivering proactive outreach and education, the nurse supports safe transitions of care, enhances patient satisfaction and contributes to improved clinical outcomes across the health system.

Qualifications

REQUIRED

  • Bachelor of Science Degree in Nursing and 4 years of clinical experience
  • OR Associate Degree in Nursing with 10 years of nursing experience
  • Excellent communication and listening skills.
  • Demonstrated assertiveness, coupled with tact and diplomacy.
  • Ability to relate well with hospital managers, physicians, and office staff.
  • Technical skill in PC based computer systems.
  • PREFERRED

  • Knowledge of Northside Hospital's medical staff and management team.
  • Work Hours:

    8AM-4PM
