Schedule: Monday – Friday | 8:30 AM – 5:00 PM
Salary: $70,000 – $80,000

We are seeking a Clinical Case Manager to join our Maitland, FL office and oversee non-skilled home care services
This is a primarily in-office position with occasional field visits as needed to ensure the highest quality of client care. You’ll collaborate with our caregivers, clients, and office team to create meaningful experiences and ensure each individual receives the care they deserve — all while enjoying a healthy work-life balance and a positive, team-oriented environment
Current RN license in the State of Florida (required)
1+ year of experience in home care or community health preferred
Excellent communication, leadership, and organizational skills
Positive, professional attitude and a strong commitment to quality care
Reliable transportation and valid driver’s license for occasional field visits
Familiarity with AlayaCare software a plus
