The On-Call Night Nurse is responsible for providing nursing care, both over the phone and in person, to hospice patient's during the evening hours when the office is closed: 1700 to 0900. This position works from home and makes patient visits, as necessary. Please note, the remote work/home location must be within the Hospice of Humboldt service area.
Essential Duties and Responsibilities
Provides direct nursing care in the patient’s place of residence.
Communicates directly with the after-hours answering service. Responds within 10 minutes of a call.
Accesses and obtains patient care report from interdisciplinary team members at the beginning of the shift.
Assesses and documents in the medical record the physical, psycho-social, and spiritual needs of each patient.
Provides home nursing care instructions to patients.
Assesses all patient/family calls in accordance with on-call procedures and makes home visits to those whose issues cannot be satisfactorily handled by telephone.
Maintains communication with the patient’s physician(s) as needed for medical authorizations, pain and symptom management, changes to the Plan of Care, and other exchanges of information.
Participates in the revision of the patient's Plan of Care.
QualificationsMinimum Qualifications
Graduate of an accredited nursing program.
Current California Registered Nurse license.
Minimum of one (1) year of experience as a professional nurse within the previous three years or equivalent experience in BSN program.
Work independently with minimal supervision.
Excellent communication skills, with ability to triage symptom management issues over the phone, while providing comfort to patients and families who may be in crises.
Demonstrated critical thinking skills and excellent clinical judgment.
Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation.
Proficient with standard healthcare technology (Windows, EMRs, laptops, email, etc.) and willingness to expand skills as needed.
Preferred Qualifications
One (1) year of hospice or palliative care experience.
CHPN certification.
