Advanced Practice Provider: Palliative Care

Position Highlights:

UW Health Northern Illinois seeks an Advance Practice Provider (APRN or PA-C) to join the Palliative Care team at UW Health Northern Illinois Hospital in Rockford, IL. Our philosophy is to provide compassionate, supportive, comprehensive care for our community.

Palliative Care Advanced Practice Provider needed to expand busy clinic-based/hospital-based program with a BC palliative care physician. Experience and certification in palliative care are preferred but will consider training select candidates. Award-winning UW Health Northern Illinois Hospital is a 330-bed Top Ten Hospital serving 12 counties in northern IL and southern WI.

Qualifications:

  • Master’s degree from an accredited PA program or completion of a master’s level advanced practice nurse program.
  • Active APRN/PA license in the State of IL and Active DEA license or ability to apply for such a license prior to or upon hire.

Job Duties:

  • Conduct comprehensive patient assessments, including history-taking, physical examinations, and functional status evaluations.
  • Diagnose and manage symptoms such as pain, nausea, dyspnea, fatigue, anxiety, and depression in patients with serious illnesses.
  • Develop and implement individualized palliative care plans in collaboration with physicians and other healthcare professionals.
  • Prescribe medications for symptom management, including opioids, antiemetics, anxiolytics, and antidepressants (depending on state regulations).
  • Provide guidance on advance care planning, including goals of care discussions, advance directives, and code status preferences.

Patient & Family Support

  • Educate patients and families about disease progression, prognosis, and symptom management strategies.
  • Facilitate discussions on end-of-life care options, including hospice referrals and Do Not Resuscitate (DNR) orders.
  • Provide emotional and psychological support to patients and caregivers coping with terminal illnesses.
  • Help families navigate complex medical decisions and align treatment plans with patient values.

Collaboration & Coordination

  • Work within an interdisciplinary team, including physicians, nurses, social workers, chaplains, and therapists, to provide holistic care.
  • Communicate with primary care providers, oncologists, intensivists, and other specialists to coordinate palliative and supportive care.
  • Assist in transitions of care, such as from hospital to home hospice or long-term care facilities

Administrative Roles

  • Document patient encounters and care plans in electronic medical records (EMR).
  • Participate in quality improvement initiatives and develop protocols for palliative care best practices.
  • Educate healthcare staff, including nurses and medical residents, on palliative care principles.
