The Our House Residential Care Facility (RCF) is a 14-room facility with a team of expert clinicians who work with people living with HIV who are having difficulty managing independent living. By tailoring nursing, social services, and therapeutic everyday activities our clients and residents are able to define what ‘living well’ means to them.

Our House of Portland a program apart of Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Neighborhood Housing + Care Program (NHCP) Housing Case Worker The NHACP Housing Case Worker role is to support our clients obtaining and maintaining stable housing. NHCP clients may experience a wide range of challenges to their wellbeing including HIV/AIDS, a history of substance use, and mental health challenges. The Housing Case Worker will serve as a liaison between clients and property management, and funding resources. Serving as an advocate and motivator while assisting clients with building the fundamental skills of responsible renting.

This is a Full-Time role working at Our House of Portland in SE Portland. The schedule for this role is Monday-Thursday 8:30am-5:00pm. This is a hybrid role 3 days in office and 1 day work from home.

The pay for this position is $34.51 which is equivalent to just over $57k a year. The NHCP Housing Case Worker’s pay increases with tenure and the top of the pay scale is $39.33. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

Who You Are

  • GED or High school equivalent

  • Current unrestricted Oregon Drivers License

  • Ability to be insured under employers’ insurance.

  • Successful experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced houselessness, persons with a mental illness and/or substance use issues.

  • Experience working with people living with HIV/AIDS (PLWHA) and/or people with disabilities, chronic illness and/or other barriers to employment. A strong understanding of issues relevant to people living with HIV, communities of color, and the LBGTQ+ community and how those issues affect or influence service delivery.

What You’ll Do

  • Assist individuals in identifying barriers to obtaining/ maintaining stable housing

  • Assist individuals in developing their own plan for defining and advancing in their path to wellness with person centered care, harm reduction techniques and therapeutic boundaries.

  • Works in collaboration with NHCP clinicians, clients’ primary care team, and community partners to provide wrap around services.

  • Transport clients using a company vehicle to medical appointments, housing-related meetings, grocery shopping, and other goal-related activities

  • Develop and maintain knowledge of community housing resources through training and independent research

  • Identify open housing waitlists and opportunities; assist clients with application processes when appropriate

Founded in 1983 and incorporated in 1985 as the Cascade AIDS Project, CAP is the oldest and largest community-based provider of HIV services, housing, education and advocacy in Oregon and Southwest Washington.
