Salary Range: $31.00 - $35.00 Hourly
Join Pilgrim Place, a mission-driven senior living community in Claremont, where compassion, clinical excellence, and teamwork guide the care we provide. We are seeking a detail-oriented and dedicated Neighborhood/Treatment Nurse (LVN) to join our team in a part-time, weekends-only role.
Under the direction of the Director of Nursing Services, this position is responsible for implementing and overseeing all physician-ordered treatments, with a strong emphasis on skin integrity, wound care management, and interdisciplinary collaboration to ensure high-quality resident outcomes.
What You’ll Do
- Perform comprehensive skin assessments upon admission, readmission, and return from leave.
- Initiate and maintain Nurse’s Skin and Wound Progress Reports when skin concerns are identified.
- Promptly notify physicians of new, worsening, or non-responsive skin conditions.
- Implement and document all prescribed treatments accurately and timely.
- Ensure all skin conditions are properly identified, photographed (as required), and documented in the medical record and Plan of Care.
- Conduct weekly skin assessments and communicate findings to the interdisciplinary team.
- Coordinate with Dietary, Physical Therapy, MDS, and Administration regarding wound management needs.
- Lead or participate in wound-related interdisciplinary discussions.
- Order medications and wound supplies as prescribed.
- Maintain accurate treatment sheets and complete required audits.
- Ensure treatment carts and supplies are clean, organized, and compliant with standards.
- Protect resident rights and present a professional image at all times.

Qualifications
What You Bring
- Graduate of an accredited school of professional nursing.
- Current, active California Licensed Vocational Nurse (LVN) license.
- Minimum of one year of experience in a skilled nursing facility preferred.
- Ability to read, write, and communicate effectively in English.
- Strong clinical assessment skills and attention to detail.
- Commitment to resident dignity, safety, and regulatory compliance.
