Job Description

We are seeking a highly motivated and experienced NDIS Support Coordinator to join our team. The successful candidate will be responsible for providing support and guidance to individuals with disabilities and their families, ensuring they receive the best possible care and services under the National Disability Insurance Scheme (NDIS).

Responsibilities:

  • Conducting assessments and developing individualised support plans for NDIS participants
  • Coordinating and monitoring the delivery of services and supports to NDIS participants
  • Liaising with service providers, healthcare professionals, and other stakeholders to ensure the best outcomes for NDIS participants
  • Providing information and advice to NDIS participants and their families on available services and supports
  • Maintaining accurate records and documentation in accordance with NDIS guidelines
  • Participating in team meetings and professional development activities to enhance knowledge and skills

Qualifications:

  • Bachelor's degree in a relevant field such as social work, psychology, or disability studies
  • Demonstrated experience in the disability sector, particularly in NDIS support coordination
  • Strong knowledge of the NDIS framework and guidelines
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organisational and time management skills
  • Current driver's license and access to a reliable vehicle

Our programs include Therapeutic Intensive Residential Care, Intensive Family Preservation, Therapeutic Foster Care, Restoration, and other associated care types. We are a registered NDIS provider and operate a social enterprise Maintenance and repairs business.
