Hope and Change is looking for an experienced NDIS Support Coordinator to join our growing team.
This is a full-time, hybrid role offering flexibility across home, our Indooroopilly office and the community. You will have the autonomy to manage your own caseload while remaining connected to an experienced team that values good judgement, collaboration, accountability and quality practice.

About the role
As a Support Coordinator with Hope and Change, you will work alongside participants to help them understand, implement and make effective use of their NDIS plans.
Your responsibilities will include:
- developing strong professional relationships with participants and their support networks
- helping participants understand their NDIS plans, funding and available supports
- connecting participants with suitable disability, health, housing, community and mainstream services
- sourcing and coordinating providers based on participant choice, needs and goals
- monitoring plan utilisation, budgets and potential funding risks
- identifying service gaps, barriers and changes in participant circumstances
- supporting participants to build their capacity to understand and manage their supports
- preparing reports and evidence for plan reviews and reassessments
- maintaining accurate and timely case notes and participant records
- communicating with families, nominees, providers, clinicians and other stakeholders
- identifying and escalating risks appropriately
- independently managing your caseload, workflow and priorities
- achieving agreed billable and utilisation expectations.
Ultimately, your role is to help participants turn their NDIS funding into practical supports, increased capacity and meaningful outcomes in their everyday lives
About you
This is not an entry-level Support Coordination position. We are looking for someone who understands the realities of the role and can work with a high level of independence and accountability.
You will ideally bring:
- a minimum of one year's experience in NDIS Support Coordination, or substantial relevant case management experience
- strong working knowledge of the NDIS and participant plan implementation
- demonstrated experience independently managing a participant caseload
- confidence interpreting NDIS plans, budgets and funding categories
- experience monitoring utilisation and identifying potential underspend, overspend or funding risks
- an understanding of plan reviews, reassessments and changes in participant circumstances
- strong written communication, case noting and report-writing skills
- the ability to build effective professional relationships with participants, families and providers
- sound judgement when responding to complex situations, competing priorities and participant risk
- strong organisational and time-management skills
- confidence working independently while remaining connected to a broader team
- an open Australian driver's licence and reliable vehicle
- a suitable home working environment and reliable internet access.
Qualifications in disability, community services, mental health, social work, human services or a related discipline will be highly regarded.
We are particularly looking for someone who is resourceful, organised, professional and accountable You follow through on commitments, communicate clearly, maintain accurate records and understand the balance between participant choice, safeguarding, funding and sustainable service delivery.
You are willing to investigate options, advocate appropriately and help participants navigate complexity rather than simply process referrals and service agreements.

- NDIS Worker Screening Check
- NDIS Worker Screening (Yellow Card)
- Working with Children Check (Blue Card)
- NDIS Worker Orientation Module
- 100 points of identification
- proof of right to work in Australia
- current Australian driver's licence.
