About the Role
We are looking for a Support Coordinator to work alongside participants to implement their NDIS plans and build capacity through strong, person-centred connections. You will liaise with families, providers, and health professionals, ensuring services align with individual goals and funding. This role is ideal for someone with experience in complex support environments and a strong understanding of the NDIS.
What You Will Bring
• Diploma in Disability, Community Services, or a related field (Preferred)
• Experience supporting people with complex needs and behaviours
• Excellent communication, planning, and coordination skills
• Confidence working across health, community, and allied services
