Job Description

Hope and Change is seeking an experienced NDIS Support Coordinator to join our growing team.

This role is suited to someone who understands the NDIS, can manage a caseload independently, and is confident working in a role with clear billable KPI and documentation expectations.

Hope and Change is a registered NDIS provider supporting people with disability to live more independent, connected and meaningful lives.

We work with participants, families, nominees, providers and mainstream services to help people understand their NDIS plans, access appropriate supports, build capacity and achieve practical outcomes.

About the role

As an NDIS Support Coordinator, you will coordinate, implement and monitor NDIS-funded supports for participants.

Your responsibilities will include:

  • Supporting participants to understand and use their NDIS plans
  • Connecting participants with suitable disability, health, housing, community and mainstream supports
  • Monitoring participant budgets, plan utilisation and funding risks
  • Maintaining accurate case notes, records and reports
  • Preparing for plan reviews and reassessments
  • Identifying risks, barriers and changes in participant circumstances
  • Working collaboratively with families, providers and stakeholders
  • Managing a caseload independently
  • Meeting agreed billable KPI and utilisation expectations
  • Attending regular team check-ins, supervision and meetings

About you

To be successful, you will bring:

  • Minimum 2 years’ experience in NDIS Support Coordination or a related case management role
  • Strong understanding of NDIS plans, funding, implementation and review processes
  • Experience managing a caseload independently
  • Proven ability to meet KPI, utilisation or billable work expectations
  • Strong written communication and documentation skills
  • Confidence managing participant budgets and plan utilisation
  • Sound judgement when responding to risks, service gaps or changes in participant needs
  • A clear focus on participant outcomes and quality practice
  • Current open driver licence and reliable vehicle
  • Suitable home office set-up and reliable internet

Relevant qualifications in disability, community services, mental health, social work, human services or a related field will be highly regarded.

Before commencement, you will need to provide:

  • NDIS Worker Screening Check
  • NDIS Worker Screenting (Yellow Card)
  • Working with Children Check (Blue Card)
  • NDIS Worker Orientation Module
  • Infection control training
  • 100 points of identification
  • Proof of right to work in Australia
  • Current driver licence
