Job Shift: AnyJOB PURPOSE:
The Navigator position is a combination of duties including Outreach and Case Management. The person occupying this position will work directly with Veterans that are chronically homeless and assist them in accessing community support systems, services, benefits, and permanent housing. The Navigator directly supports the homeless Veteran community and collaborates with multiple service providers meeting regularly to triage, staff, and house the most vulnerable unsheltered homeless Veterans. The Navigator will be the lead on assisting the Veteran population with a history of homelessness. The Navigator will be the first point of contact with the affected veterans and building trust and rapport will be crucial for the role.
The Navigator will have a caseload of approximately 20 Veterans and serve as the primary advocate for the Veteran for 1-2 years.

- - - - Conduct proactive outreach and networking activities to identify and engage homeless and at-risk Veterans in community settings such as stand-downs, encampments, shelters, transportation hubs, and other locations frequented by individuals experiencing homelessness; actively participate in Veteran-focused coalitions and community groups, deliver presentations to service providers and community partners throughout the service area, and promote the mission, services, successes, and community impact of the Navigator and Supportive Services for Veteran Families (SSVF) programs.
- - - - Conduct initial eligibility screenings to determine Veteran qualification for program services, complete comprehensive intake assessments and required case management documentation, and accurately collect, verify, and maintain participant information in accordance with program policies and compliance requirements.
- - - - Assess the needs, strengths, barriers, and goals of Veterans and their families; develop individualized housing and service plans in collaboration with participants, establish measurable action steps to address identified needs, and provide ongoing case management support to promote housing stability and self-sufficiency. Regularly review and update housing and service plans to reflect progress, completed objectives, changing circumstances, and emerging needs to ensure services remain aligned with participant
- - - - Coordinate, monitor, and document service delivery to ensure progress toward housing and service plan goals, including comprehensive tracking of Veteran activities, referrals, outcomes, and case management interactions; conduct and document housing inspections as required; assist Veterans in obtaining identification cards, DD214s, birth certificates, Social Security cards, and other essential documents needed to access benefits, healthcare, employment, community resources, and permanent housing.
- - - - Provide intensive, hands-on support and advocacy for Veterans experiencing chronic homelessness who may face significant barriers to self-sufficiency, including limited life skills, behavioral health challenges, or difficulty navigating complex systems. Assist Veterans with completing housing applications, benefit applications, and other required paperwork, and when necessary, accompany and transport participants to appointments, housing viewings, and service provider meetings based on individual needs, abilities, and level of support required.
- - - - Maintain accurate, timely, and confidential client records in HMIS and other required data systems, ensuring all services, contacts, referrals, assessments, housing activities, and case management interventions are documented with the appropriate date, time, and level of detail. Prepare and maintain comprehensive case notes that clearly reflect participant progress, barriers, service plan updates, housing search activities, and outcomes, while ensuring compliance with program requirements, funding regulations, and documentation standards.
- - - - Conduct regular home visits and community-based follow-up contacts to monitor housing stability, assess ongoing needs, reinforce progress toward service plan goals, and provide supportive case management services
- - - - Meet regularly with the Program Director to review caseloads, discuss client progress and barriers, and develop strategies to improve outcomes. Provide recommendations for program improvement, support quality assurance efforts, and prepare reports and case updates as requested.

ESSENTIAL QUALIFICATIONS:
EDUCATION:  Bachelor’s Degree in Social Work, Sociology, Psychology or related field.
EXPERIENCE:  1+ years case management experience; 3+ years preferred. Experience working with the homeless veteran population with outreach efforts targeting the most vulnerable, unsheltered homeless Veteran households preferred.  Knowledge of VA Programs, facilities, and community preferred.
ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion.
LICENSES: LMSW, LBSW, LMFT preferred. Driver’s License with clear record.
VEHICLE: Must have daily use of a vehicle without prior notice.
OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
